SignatureFactory HTML Order Form

Notes:

• Cost for Signature Font is $9.95 (font will be returned by e-mail)
• For credit card orders, include Card Number, Expiration Date, Card Type, Cardholder Name, and Cardholder billing address.  Visa, Mastercard, and American Express are accepted.
• e-mail address is required; Signature font will be returned by e-mail.
• If desired, signature font can be sent on a diskette by U.S. mail. Add $3 shipping & handling for U.S. Addresses or $6 shipping and handling for non-U.S. addresses.
• Place your signature, initials or whatever you want converted to a font below or on an attached page. If multiple signatures are present put an arrow next to the one you want made into a font. Only one signature will be converted to a font per order. Make sure area around signature is free from stray marks, etc. which you do not want included in your signature (even faint marks will show). When folding order for mailing, fold signature area in last so that it does not touch other text.
• For best results send your order in by mail. Faxed signatures can give unpredictable results. If faxing write your signature large and use a fax machine that supports Class 2 type faxes (fine resolution). Only credit cards are accepted for fax orders.
